Forms Handler PHP - empty fields?

Hi guys

I have put together an Order form using the Form Handler PHP.
Everything works ok except it leaves some fields blank after submission.

I have gone through it all making sure the forms textbox inspector
corresponds with the form label inspector etc.
I have been pulling my hair out all day trying to work out why some fields
are empty after submittion.
Can anyone help me.
The fields that are left empty are:
Part No.
Card type
Expiry date
Name on card
Address on card

I even deleted the combo boxes and replaced them with just simple forms edit
fields incase it was a glitch.

One thing your labels all have in common is the use of a space. It may be
that you are also using a space in your field names. Try substituting an
underscore.
