Form Handler

I am unalble to create Form Handler form in my layout. I click NOF Forms Handler - forms Handler, then draw a box in my layout in page view. I get the NOF forms Handler window to create a Form Handler suite. I create a new name and click OK and a Forms Handler windows shows up with nothing in it. My only option is to click the X in the uppper right corner. Nothing is created. I tried to use an existing name that I created and I get the same thing. I have never been able to create a Form Hangler form. Can someone please help me with this issue.
