My publish settings for my site change almost everytime I go to publish and need to upload to server.
I want to publish - html> assets> images> autogen ...NF 12.0 adds different folder/file structure. What gives?

12 uses by site style as the default.
Change the default to by asset type (see this gotFusion tutorial for details) http://www.gotfusion.com/tutorials/tut.cfm?itemID=181
You will also want to clear off your work files/folders (see this gotFusion tutorial for details) http://www.gotfusion.com/tutorials/tut.cfm?itemID=4029
You will also want to clean of your hosting space also before you upload to remove all orphan files/folders

Mike, I've done all you suggested - NF 12.0 is creating the index.html file under the autogen directory...? Not sure how this has happened?
Goto site view.
From the menu select Tools > Re-optimize.
Go to publish view.
Select a different publish method.
Close Fusion
Open Fusion
Reselect the publish method of your choice.
If index.html is still not in root go back to publish view, open the assets folder, right click on the index.html file, select "cut" Go to the root folder. Right click and select "paste" Index.html will be back in root.

Sorry to butt in here.... Mike, I've seen you refer to this a thousand times, and I'm now (yeah, just now) wondering: are you recommending to use Local Publish and then upload your site via FTP from the Local Publish folder as a standard practice? Rather than the NOF built-in FTP/Publish?
