HELP! Inserting forms

So it took me awhile (since 2013!) to figure out that the form I created in a previous version - which was working nicely - is no longer working on Fusion 12...
and I don't know how to create one and insert it.

Can someone "dumb it down" for me?

It was a simple "contact us" form which sent the form contents to my email.

How to "do it" depends on how you made your form in the first place as there are so many possibilities and options.
