Issues with Form handler

Hi folks,

I'm still trying to set up a form my website and never done one before using formhandler.php. Im using NO15, I have created the form, filled out the form properties as much as i can but when I test the submit button, I'm getting the error 500. pics attached for the error and my properties box.
I contacted GoDaddy about the SMTP info and they indicated to use localhost as I use the CPANEL.
If anyone has run into this before or can point out any possible errors in my properties panel, I would greatly apricate it.
