Forms with captcha?

This looked interesting to me, But the first address goes to a web site developer, the second does not work.

The previous postings are all 6 years old. Charles Edmond's site used to produce a number of very helpful applications which many here used. He's not a web site developer but a purveyor of applications. Nancy O hasn't been on these forums for many years now, but her site is still up and has a number of resources and tutorials, although she dropped Fusion some years ago.
