    I can't comment on incorporating a Microsoft Office Spreadsheet into
    your web page, but I did this once in PHP and it was pretty easy to do.
    I don't know if it's better.... I just know it works.

    Default Creating a survey form

    Hello,
    I am trying to create a form that has 10 questions and each question has an
    answer from one to ten. After taking the survey there would be a total
    score which indicates what action you need to take. I have tried to create
    this using the ActiveX plugin for Microsoft Office Spreadsheet 9.0 but can't
    get the information that I have entered to save. Can someone tell me what I
    am doing wrong or suggest a better way to create this survey?
